FAQs - OFFICE POLICIES
1. We accept most major health inusrance plans.
2. You must have a referral from your Primary Care Physician.
3. Cancelations must be given at least 24 hours in advance of your appointment.
4. Co-Pays are due at the time of service.
5. No narcotics will be prescribed on your first visit and you will be required to sign a release and contract regarding any narcotic prescription and terms of use.
